We're happy you've chosen SIAST as your new workplace!

To begin your career with SIAST, we ask that you complete the forms below and bring the completed copies with you on your first day of work or to your scheduled meeting with Human Resources. This will reduce the amount of time needed to complete the necessary paperwork on your first day. To complete these forms you will need to know three things, but don't worry all this information is found in your offer letter.

The three things you need to know are:

  • The location that you will be working (Saskatoon, Regina, Prince Albert, or Moose Jaw)
  • If you are a member of the Professional bargaining unit, Academic bargaining unit, or if you are an Out of Scope Employee
  • The type of position you are going into (Full Time, Part Time, End Dated, or Casual).

On your first day or for your scheduled meeting with Human Resources, please bring the following:

  • SIN Card
  • Photo ID
  • Void Cheque
  • Transcripts
  • Certificate, if needed (Journeyperson's, Professional Designation, etc)

    • Bring a void cheque. If this is possible, you then will only need to fill out your name and campus location at the top of the form and sign your name at the bottom.
    • If you DO NOT have a void cheque, please take this form to your bank and have them complete it with your banking information. You then will need to fill out your name and campus location at the top of the form and sign your name at the bottom.

Benefits

Forms for the benefit programs will be filled out on your first day of work. SIAST's dental plan is located through Public Employees Benefits Agency (PEBA). The intent of the Plan is to promote good dental health by reducing the costs for preventative, routine and major restorative dental work. The insurance carrier for the Plan is Great West Life. The Administrator for the Plan is the Public Employees Benefits Agency, Saskatchewan Finance.

Fulltime SIAST employees are eligible for claiming dental coverage starting on the first day of the month following 6 months of continuous employment, while part time and casual employees may become eligible for benefits after meeting certain eligibility requirements.

Making Dental Claims:

PEBA will send you a Green wallet card with your plan number (57447) and ID number (which is your SIAST employee number). Present this information to your dentist's office, and they will directly contact PEBA for claim payments. 
If you have claims from the first six months of employment, you will have to directly make the claims to PEBA.

SIAST's Group Life Insurance is located through Public Employees Benefits Agency (PEBA). The Great West Life Assurance Company provides claims adjudication and benefit payment services to the Plan.

SIAST's health plan is handled through Great West Life. To find out more about the plan, please visit their website. Access to your benefits information has never been easier, as you are able to view all your information online.

How to view your information:

  • Visit www.greatwestlife.com
  • Click on GroupNet for Plan Members, found on the right side of the screen.
  • If you are a new employee click on, “New User? Register Now...”
  • You will need to have this information to fill out the registration form:
    • Plan Number: 57447 (for all SIAST in-scope staff and OOS staff bands 1-9)
    • ID Number – which is your SIAST employee number – not including zeros at the beginning
    • Your date of birth
    • Date of birth of one of your dependents, if applicable
    • Your postal code
    • Your e-mail address
  • You will then get to choose a user name and password, which will be used for each additional login.

 On Great West Life's site you can:

  • Complete and print personalized claim forms
  • View your claim status and Explanation of Benefits for the past 24 months
  • Sign up for Direct Deposit Claim Payments – claims paid directly into your bank account
  • Look up common group insurance terms and Frequently Asked Questions
  • See the Health & Wellness section for topics that are important to you
  • Personalized health evaluation tool: Modifiable Health Risk Assessment
  • Access expanded coverage information quickly and easily

For in-scope employees, Long Term Disability (LTD) is held through Saskatchewan Government and General Employees' Union.

Public Employees Pension Plan (PEPP):  http://www.peba.gov.sk.ca/pepphome.htm

SIAST's pension plan is held through Public Employees Pension Plan (PEPP). It is a defined contribution pension plan administered by PEBA with more than 100 participating employers and over 49,000 members. PEPP employers include the Government of Saskatchewan, certain Crown corporations, agencies, boards and other institutions.

PEPP will send out a package to the home with new employees when the new employee is eligible for the pension plan.
